The Toronto D2 Gastrectomy Program is designed for surgeons who operate on a high volume of patients with gastric cancer (on average at least 6 cases per year). To register for the Toronto D2 Gastrectomy Program held at Mount Sinai Hospital on June 15th, please complete the following form:<\/p>\n

Upon completion and approval to attend, the CPD office will forward you a link to complete your registration. For further information and assistance, please contact the CPD office at facmed.registration@utoronto.ca<\/a>. The course directors reserve the right to cancel registrations that do not fulfill the volume criteria.<\/p>\n

Please note all accepted learners will be forwarded a survey link that must be completed as part of the workshop curriculum.<\/p>\n

***Delegates attending the June 14 & 15 D2 Gastrectomy Program must register no later than February 1, 2018.<\/small><\/div>\n

All registration fees are subject to applicable 13% HST<\/p>\n

Cancellation<\/h2>\n

Refunds will not be issued for cancellations received after May 1, 2018<\/strong>. A processing fee of $500 will be retained on all cancellations. Requests for cancellation must be made in writing. Registrations are not transferable.<\/p>\n

The University of Toronto reserves the right to cancel events. Registrants will be notified at the earliest possible date in the event of a cancellation. Registration fees for events cancelled by the University will be refunded; however, the University will not be liable for any loss, damages or other expenses that such cancellations may cause.<\/p>\n","protected":false},"excerpt":{"rendered":"
