Participants must have completed 20 cases operating or assisted per year on average to be eligible for this course. Registrants will be required to complete the survey below upon registration. The committee reserves the right to cancel registrations that do not fulfill the criteria.

Cancellation

Refunds will not be issued for cancellations following

• SUR1821J – June 7, 2018 – no cancellation refund after April 16th; early bird ends March 7, 2018

A fee of $850 will be retained on all cancellations. Requests for cancellation must be made in writing. Registrations are not transferable.

The University of Toronto reserves the right to cancel events. Registrants will be notified at the earliest possible date in the event of a cancellation. Registration fees for events cancelled by the University will be refunded; however, the University will not be liable for any loss, damages or other expenses that such cancellations may cause.
