The Toronto D2 Gastrectomy Program is designed for surgeons who operate on a high volume of patients with gastric cancer (on average at least 6 cases per year). To register for the Toronto D2 Gastrectomy Program held at Mount Sinai Hospital on June 15th, please complete the following form:

Upon completion and approval to attend, the CPD office will forward you a link to complete your registration. For further information and assistance, please contact the CPD office at facmed.registration@utoronto.ca. The course directors reserve the right to cancel registrations that do not fulfill the volume criteria.

Please note all accepted learners will be forwarded a survey link that must be completed as part of the workshop curriculum.

Cancellation

Refunds will not be issued for cancellations received after May 1, 2018. A processing fee of $500 will be retained on all cancellations. Requests for cancellation must be made in writing. Registrations are not transferable.

The University of Toronto reserves the right to cancel events. Registrants will be notified at the earliest possible date in the event of a cancellation. Registration fees for events cancelled by the University will be refunded; however, the University will not be liable for any loss, damages or other expenses that such cancellations may cause.
